Please note when filling out these forms that we are a separate entity from the City of Horn Lake. So, we may not provide service to your address even if you are moving/living in Horn Lake.
After filling these forms out and submitting them, if you have not received contact within 24-48 hours (weekends are the exception) please call the office at

- If you are wanting to start service within our service area, please fill this form out, and either drop it by the office, or email it to email@example.com.
- If you are moving out of our service area, please fill out this form. Either, return it to the office or email it to firstname.lastname@example.org
- If you are a landlord looking to start temporary services, please fill this form out. You can drop it by the office or email it to email@example.com. This form allows the landlord 4 weeks with our services, without having to start an account.
- ACH form. Please do not fill out digitally and email. This particular form needs to be filled out and signed with ink. It can be mailed to us, or you can drop it off in person at the office. See the homepage for P.O. Box and office locations.
